We have a team of trained support staff who provide a high quality service in line with Domiciliary Care Agency Regulations 2002 of the National Care Standards.
Each service user has a key-worker whose role is to co-ordinate the service users’ individual support plan, ensuring that each service user’s needs are understood and appropriate support offered. Service users are consulted to ensure they feel comfortable with their key-worker.
Our Support Plan Approach
To ensure service users’ daily needs are adequately met, support plans will be developed in partnership with the service user and all the people who are important to each person. The support plan details what work will be done together to reach the specified and agreed goals and highlight to staff what to do as well as how to do it, in the way the individual chooses.
Person-centered Planning is at the core of our philosophy
We use person-centered planning to set out how we will support our service user’s. A Person-centred Plan is important to help you respect yourself and others, live safely, eat healthily and be active within your immediate and wider community.
We will make this plan with you based on your own aims and what is important to you so that you can build your skills and get the support you need, when you need it. We’ll also review how things are going and work with you continually to update your goals and make good choices.
